Why do you get brain freeze from cold drinks

Brain freeze is the short, sharp headache many people get when they drink something ice cold too quickly or eat ice cream too fast. The pain often appears suddenly in the forehead, behind the eyes, or at the top of the head, and it can feel intense even though it usually lasts only a short time. The phenomenon is also called cold stimulus headache or sphenopalatine ganglioneuralgia.

Although brain freeze can feel dramatic in the moment, it is usually harmless. It happens because of a rapid physiological reaction in nerves and blood vessels when cold touches the roof of the mouth and upper throat. The body interprets the cold signal as pain from an area close to the head, which is why the pain is often felt as a headache.

Cold touches the roof of the mouth
When you drink ice cold liquid or eat ice cream quickly, the cold comes into contact with the roof of the mouth and the upper part of the throat. This area contains many nerves and blood vessels, and it reacts quickly to temperature changes. The faster and stronger the cold exposure is, the greater the chance of brain freeze.

The lining of the palate is thin and sensitive, and nerve branches beneath it send signals to the brain. When the temperature drops suddenly, cold receptors and pain receptors in the area become activated. This creates a strong sensory signal that the body has to interpret quickly.

A key part of the explanation involves the trigeminal nerve. This is one of the major sensory nerves of the head and face. It detects temperature, touch, and pain from the face, mouth, teeth, sinuses, and parts of the head.

When the palate is exposed to intense cold, the trigeminal nerve sends signals to the brain. The problem is that the brain does not always identify the exact origin of the signal. Pain from the palate can therefore be interpreted as pain in the forehead or head. This is called referred pain.

Referred pain means that pain is felt somewhere other than where the stimulation actually occurs. The same principle can be seen in other situations, such as pain from the jaw, neck, or sinuses being experienced as a headache.

Blood vessels react quickly
Cold affects not only the nerves, but also the blood vessels. When the palate cools rapidly, blood vessels in the area may constrict. They can then quickly dilate again as the temperature normalizes. This rapid change in blood vessel diameter may contribute to the pain sensation.

Why the pain comes so quickly
Brain freeze often appears within seconds. This is because the temperature change in the mouth happens very quickly, and the nerves in the area are fast at sending signals. The body is designed to respond efficiently to potentially harmful stimuli, and extreme cold is registered as something important.

At the same time, the palate is anatomically close to several structures involved in pain perception in the head. When signals travel through the trigeminal nerve, the pain may feel as if it is located in the head, even though the stimulation started in the mouth.

The reason the pain fades quickly is that the temperature in the palate normalizes when the cold food or drink is removed or warmed by the mouth. As the cold signal decreases, nerve activity also settles down.

Why some people get it more easily
Not everyone gets brain freeze equally easily. Some people can eat ice cream quickly without any problem, while others feel pain after only a few sips of cold drink. This may be due to individual differences in trigeminal nerve sensitivity, blood vessel response, and how quickly the palate cools.

People who have migraines or are prone to headaches may be more likely to experience cold stimulus headaches in some studies. This may be because the pain pathways in the trigeminal system are already more sensitive. This does not mean that brain freeze is dangerous, but that the nervous system may be more reactive in some people.

how quickly you eat or drink
how cold the food or drink is
how much cold reaches the palate
individual nerve sensitivity
tendency toward migraine or headache

How to stop brain freeze
The simplest way to stop brain freeze is to warm the palate again. Since cold exposure to the palate is the trigger, the pain often fades when the temperature returns to normal. This can be done by pressing the tongue against the roof of the mouth, drinking something lukewarm, or simply stopping the cold food or drink for a few seconds.

Brain freeze can also be prevented by eating cold foods more slowly. When cold is introduced gradually, the tissue in the mouth has more time to adapt, and the nerve activation becomes less sudden.

Strategies that may help include:
eat ice cream more slowly
take smaller mouthfuls
avoid letting ice cold drinks hit the palate directly
press the tongue against the palate when pain starts
wait a few seconds before continuing

When to pay attention
Brain freeze is usually harmless and short lived. However, it should be distinguished from headaches that occur without a clear cold trigger, last for a long time, or feel different from usual. A cold stimulus headache should normally go away quite soon after the cold exposure stops.

It may be worth paying attention if the headache:
lasts long after the cold is gone
is unusually severe or different from before
occurs without cold food or drink
is accompanied by neurological symptoms
occurs with visual disturbances or severe dizziness

Such symptoms do not necessarily mean that something serious is present, but they do not fit typical brain freeze and should be assessed more carefully.
