Early-stage rehabilitation following anterior cruciate ligament (ACL) reconstruction with a bone–patellar tendon–bone (BPTB) graft requires a careful balance between restoring muscle strength and minimizing knee pain, effusion, and excessive joint stress. Heavy-load resistance training is traditionally recommended to address quadriceps weakness, but it is not always tolerated in the early postoperative phase. This randomized controlled trial investigated whether low-load blood flow restriction training (LL-BFRT) provides comparable outcomes to heavy-load resistance training (HLRT) during early rehabilitation.
