Stroke is one of the leading causes of long-term disability in adults, and many individuals experience persistent impairments in gait, endurance, and independence. Traditional stroke rehabilitation often delivers relatively low training intensity, despite growing evidence that higher exercise doses may be critical for neurological recovery. A study published in Physical Therapy in December 2025 presents the protocol for the Intensive Stroke Cycling for Optimal Recovery and Economic Value Trial, a randomized clinical trial designed to evaluate the effectiveness and cost-efficiency of intensive cycling as part of post-stroke rehabilitation.
